Each status links to the 277 response and the next step already queued.
A claim nobody checks is a claim that can quietly miss its filing limit. Working every open claim on a schedule keeps accounts receivable from aging out.

What claim status tracking means in practice.
Claim status tracking is asking the payer where every unpaid claim stands, on a schedule, and acting on the answer before the claim runs out of time.
Most clinics already know this matters. The trouble is the method: a portal per payer, a hold queue for the rest, and a spreadsheet that is accurate for about a day. Claims that are not chased tend to be the ones that quietly hit their filing limit.
2care sends an electronic claim status check (an X12 276) for every open claim, reads the 277 that comes back, and turns the status code into a next step. Where the electronic answer is empty or vague, it calls the payer and gets a person to say it.

61 to 90 days
Industry guidance treats a claim over 90 days as at real risk of never being paid.
Every bucket has a reason
Claims are grouped by days since filing, and each group shows its status mix, so you can see whether 61 to 90 day claims are pending or simply missing.
Timely filing gets its own line
Claims whose payer limit is close are counted separately, because those are the ones where a week of delay turns into a write-off.
Numbers you can click
Each figure opens the list of claims behind it, with the latest status response and who has the next action.
How an open claim gets worked.
The cadence is yours to set per payer. A typical setup looks like this.
First check after the acknowledgment
Once the 277CA confirms receipt, the claim gets its first status check about 10 to 14 days later, when most payers have something to report.
Repeat on a schedule
Claims still in process are checked again every 7 days, or sooner for payers you know run slow. Each response is stored with its date.
Act on the answer
Paid claims close out. Pended claims get the requested records sent. Denials move to denial recovery. Claims the payer cannot find are refiled.
Call when the record runs dry
If the electronic answer stays empty or keeps saying the same thing past your threshold, a payer call is placed and the rep's answer is logged.
Escalate before the limit
Any claim within 30 days of its payer's timely filing limit goes to the top of your billing team's list with everything known so far.

A 277 response is a category code, a status code and sometimes a free text note. Each claim gets read in that order and given one plain instruction.

What practices ask before switching off the spreadsheet.
It sends a standard X12 276 request for each open claim through your existing clearinghouse connection and reads the 277 response. When that answer is missing or unclear, it checks the payer portal or calls the payer.
You set the cadence per payer. A common pattern is a first check 10 to 14 days after the acknowledgment, then weekly until the claim is paid, denied or fixed.
Every claim carries its payer's filing limit. Claims within 30 days of that limit are flagged to your billing team, and claims the payer has no record of are refiled with proof of the original submission.
Yes. When an electronic check cannot settle a claim, 2care calls the payer, works the phone menu, waits on hold, asks a set list of questions and logs the rep's name, reference number and date.
On the claim in your PMS, across 95+ integrations and EHRs. Status, dates, reference numbers and next steps are written where your team already works, plus a daily digest of exceptions.
Tracking stops and denial recovery takes over. The reason codes travel with the claim, so the next person, or the next automated step, starts from the payer's own explanation instead of a blank note.
